Parent/Guardian Consent
I am the parent or legal guardian of the minor named in this registration, or I have the legal authority to provide consent for this minor's participation.

I have read the information concerning the nature and physical requirements of the Eucharistic Procession and give permission for the minor to participate under appropriate adult supervision.

I give permission for my child/ren or ward to participate in the Eucharistic Procession from Sts. Peter & Paul Church to Co-Cathedral of St. Theresa on October 3, 2026.

I have read and agree to the conditions above and give permission for the minor(s) named in this registration.

ASSUMPTION OF RISK AND RELEASE
I understand that participation in the Eucharistic Procession involves ordinary risks associated with outdoor walking and public events, including but not limited to fatigue, falls, uneven surfaces, weather conditions, heat, sun exposure, dehydration, interaction with pedestrians or traffic, and other reasonably foreseeable risks.

I confirm that I am voluntarily choosing to participate and that I am responsible for determining whether I am physically able to participate safely.

To the fullest extent permitted by law, I voluntarily assume the ordinary risks associated with my participation in the procession.

I understand that the parish, Diocese, clergy, employees, volunteers, procession marshals, organizers, participating organizations, and their respective representatives will exercise reasonable care in conducting the event but cannot guarantee the personal safety of every participant or prevent every accident, injury, illness, loss, or unforeseen circumstance.

To the fullest extent permitted by applicable law, I agree not to hold the organizers and affiliated entities responsible for injury, illness, loss, or damage arising from the ordinary and inherent risks of my voluntary participation, except to the extent liability cannot legally be waived or excluded.

I understand that this acknowledgment does not waive any rights that cannot lawfully be waived.

By submitting this registration, I acknowledge that I have read, understood, and agree to the statements, terms, and conditions in this registration form, including the ASSUMPTION OR RISK AND RELEASE.
